Covid-19 Self-Pay Policy
All Self-Pay testing must be accompanied by a Medical Visit that varies in complexity from $99-$249
- Lab Based PCR Test (24-72 Turnaround time) $125
- Antigen Rapid Testing – $75
- Molecular Rapid Testing – $100
- BWI Location Only
- Rapid PCR Testing – $200
- Rapid PCR Combo Flu/RSV/Covid – $250
No Evaluation and Management Fees for Surveillance Testing, e.g. Testing for School, Work, Travel.
All sick visits require an Evaluation and Management Visit with a provider, which can be billed to insurance or paid up front – $99 for straight forward, $149 for low complexity, and $199 for moderate complexity.
New Patients – What To Bring
New patients to our practice are requested to complete a New Patient Questionnaire and a Registration Form prior to their first visit. You can click here to download and print the form, and fill it out at home. Make sure to bring the filled out form with you! You can print out the New Patient Questionnaire and Registration Form, or ask that it be mailed to you if you wish. Additionally, you will need to bring your insurance card and a picture identification card. Please also request your prior medical records if possible and bring them with you to your first visit. Prescription Refills
You can request all refills from your pharmacy or via your patient portal. The pharmacy will in turn fax the refill request to your doctor for authorization.
Inclement Weather
In the case of inclement weather please contact our office before coming to your appointment. We consider the safety of our patients and our staff, and evaluate the condition of the roads and the degree of icing of the parking lot before we decide on the hours that Centennial Medical Group will be open each day. There is always a physician available by phone to care for emergencies even in the event of inclement weather.
Hospital Admissions
When necessary, patients of the Centennial Medical Group are admitted by the physicians of the practice to Howard County General Hospital, a member of Johns Hopkins Medicine, in Columbia, MD. Thank you for your adherence to these policies!
