Centennial Medical Group (CMG) offers a complete range of services and tests for our patients. Access Medical Forms and your Patient History by logging into the Patient Portal.
Patient Portal Instructions:
- In Patient portal invitation was emailed to the email you provided
- Click on Link provided
- An Account Information box will appear, enter your date of birth and click Activate Account
- Another Account Information box will appear, in this box, you will need to generate a username and password (something that you will remember) and select a security question and answer
- Click Update Account
- Now log in using the username and password you generated and click Log In
- Once you log in, you must use the Previous and Next buttons to navigate your registration
- Update any information that needs to be updated and click on Save Information at the bottom of the updated page
- You MUST sign at the end to view your medical record
